6) University fee 10,600/- paid through D.D. in favour of the Registrar, Dr. YSR University of Health Sciences, A.P., Vijayawada (All India Quota candidates only)

7) College Tuition Fee Rs.15,000/- Per Year to be paid at Guntur Medical College, Guntur Union Bank of India (through Challan) (All India Quota candidates & State Quota candidates)

8) Study Certificates from 6th to Intermediate (originals)

9) Community Certificate issued by Competent authority (Below 3 years)

10) Income Certificate of the parent by Competent authority (Below 3 years)

11) Xerox copy of Student Aadhar

12) Xerox copy of Parent Aadhar

13) Passport size photos — 6

The students has to submit two(2) sets Xerox copies of all the above documents.
