University fee 10,600/- paid through D.D. in favour of the Registrar, Dr. YSR University of Health Sciences, A.P., Vijayawada (All India Quota candidates only)
College Tuition Fee Rs.15,000/- Per Year to be paid at Guntur Medical College, Guntur Union Bank of India (through Challan) (All India Quota candidates & State Quota candidates)
Study Certificates from 6th to Intermediate (originals)
Community Certificate issued by Competent authority (Below 3 years)
Income Certificate of the parent by Competent authority (Below 3 years)
Xerox copy of Student Aadhar
Xerox copy of Parent Aadhar
Passport size photos — 6
